1)  You will need to gather the following information in order to request a quote:

Current Census (should include Date of Birth, Gender, Coverage Tier (E, EESP, EECH, Family), Plan Option (if more than one option available), and Zip Code)

Current Summary of Benefits

Current Rates

Renewal Rates (if available)

SIC Code

 

2)  Submit the information one of the following ways:

Utilize our online Quote Request Form

OR

Utilize our RFQ FORM and submit email the information to rfq@vodesonline.com 

 

3)  Once we receive this information we will obtain a "street" rate quote based on demographics.

 

4)  One of our consultants will contact you to review the quote and explain self-funding in more detail.

 

5)  If you decide to move forward with the quote we will require your employees to complete an online medical questionnaire to finalize rates.
